It is strongly recommended that you have a regular yoga practice for a minimum of one year prior to applying for this study program.

APPLICATION: Please print or type answers.  Application may be brought into the studio when you come in for your interview or mailed.  Applications must be received by July 15, 2010 for the July program.  Interviews must be completed by July 20, 2010.  The interview is for you to ask questions and for us to see if we are a good fit for each other.

I hereby declare and agree that I am committed to completing this training by attending every weekend workshop (or making arrangements to make-up a weekend you may have to miss--a maximum of ONE workshop weekend can be missed and made-up.  However, this may delay completion of your program by several months.)  I also agree to pay the amount of $2,850 (if prepaid in full) in tuition to Yoga Prairie, LLC.   See Payment schedule.  Additional costs involved: Books, lodging & meals if traveling.   (Karen will provide assistance in your application for Yoga Alliance registration.)

I hereby commit to the 200+ Hr. Yoga Study program as presented by Karen Soroko, E-RYT, director of Yoga Prairie's in-depth 200 hr. yoga study program. (Please sign below).  I understand this is a legally binding instrument upon written acceptance of the student unless cancelled pursuant to the Buyer's Right to Cancel (MN 141.265 Subd 2(2))*

Please keep a copy and send the original with application to Yoga Prairie, LLC, Karen Soroko, E-RYT, 8783 Columbine Rd.,  Eden Prairie, MN 55344 with your deposit of $85 plus 1st month's tuition for the YTT program. Contract must be received by program start.  Deposit (in full) will be returned if you are not accepted into the program. 

*Buyer's Right to Cancel:  If you apply for the Yoga study  program and choose to withdraw at any point of the program, there will be no refund of fees paid except for the following:  If you are registered and cancel within 30 days of the beginning of the start date, you will be refunded your first payment minus a $85 registration fee. 

• Each student must be notified of acceptance/rejection in writing (MN 141.271 Subd. 1a). 

• If a student is rejected, all tuition, fees and other charges shall be refunded (141.271 Subd. 1a)

• If a student gives written notice of cancellation after five business days of the execution of the contract (when you sign & return it to Karen), a complete refund is given. 

• Notice of cancellation shall clearly state the date executed by student .

• Notice of cancellation shall be acknowledged in writing within 10 days of receipt of such notice.

• Refunds will be forwarded to student within 30 business days of receipt of such notice.

• Notice of cancellation must include the name of the program, date of your signing the cancellation.  There is a form to be filled out at the end of the syllabus.  If you need one, just ask.

Total Hours of Classroom instruction:  175 required - we provide well over that number of hours.
